If you are applying for coverage over the Guaranteed Issue (GI) amount or if your enrollment occurs more than 31 days after you (or your dependent) are first eligible, your policy requires Evidence of Insurability (EOI) be submitted and approved before coverage is approved for the higher amount. This article will walk you through completing your online EOI with Mutual of Omaha. You will receive an email to your primary email address in Employee Navigator that will provide you with your Group ID Number. When starting, you only need to enter the last 4 characters of the Group ID Number, ABCD, in the example below.
- Please Note: The email will also tell you if anyone else, like a spouse or eligible dependent, needs to complete EOI. Please review the Applicant Type carefully and include anyone who is required to complete EOI.
- The first step in the EOI process requires you to fill out information about Bedford City School District. You will be provided with your employer's information. Please note that subgroup numbers are not required, and if you are not provided a subgroup number, please leave it blank.
- After completing the Employer information, you will need to fill out information about yourself and your employment.
- Following your information, if applicable, you will complete information about your spouse and any eligible dependents who also require EOI.

During the next part of the process, you must input your current amount of coverage as well as the additional amount you are applying for.

Please Note: You will need to use the benefit summary you were provided to calculate the Additional Amount of Insurance. To do this you will need to use Total Amount of Insurance Requested (listed as "Pending" on your benefit summary, and subtract your Current Amount of Insurance. The difference is the Additional Amount of Insurance.
- The next steps of the process require you to answer medical questions for anyone who is subject to EOI. If there is any missing information it will appear with a red X next to it on the left-hand menu.
- Finally, when you have completed all of the required information you, and any other members subject to EOI, will need to electronically sign the EOI form and click Submit. Mutual of Omaha will contact you when they have completed the review of your EOI. No further action is required from you or any covered dependents.
