Dependent Eligibility Audit: Required Documents

As part of our ongoing efforts to control costs and to keep our enrollment records up to date, Bedford City School District has contracted with BMI Audit Services to perform relationship verification for dependent(s) enrolled in our medical insurance programs. 

If you cover one or more family members on a Bedford City Schools medical plan, your participation in the verification process is mandatory.

Documentation of your covered dependents' (spouses, children) relationship to you will be required. Covered spouses and their employers may also be required to complete a separate insurance inquiry form.  

If you cover your spouse:

  • Your spouse and their employer must complete and sign a Spousal Coverage Form. Note the Spousal Coverage Form will not be required for BEA members hired before 7/1/2007 or OAPSIE members hired before 1/1/2008. All other requirements (below) will still apply.
  • You must provide a copy of your certified marriage certificate.
  • You must provide one additional, joint document. Joint documents must be dated within the last 90 days and be addressed to both you and your covered spouse. Examples include a utility bill, mortgage statement, insurance statement, or last year's income tax form.

If you cover one or more children:

  • You must provide a copy of each child's certified birth certificate or court document.
  • If the child is your stepchild, you must provide a copy of their certified birth certificate, plus a copy of your certified marriage certificate and one joint document to verify the natural parent's relationship to you.

If you do not have a required certificate or document copy, please order it immediately. The vital statistics website (https://www.cdc.gov/nchs/w2w/index.htm) can help you determine how to obtain documents, or you can visit the county or town clerk’s office. Most court records can be obtained directly from the courthouse. Please note that you will not be reimbursed for costs associated with obtaining new copies.

If you are not enrolled in the District's medical insurance, or if your medical coverage only includes yourself, this audit does not apply.
